What does Guardian insurance do about health insurance claims that are filed after the policy has terminated if the date of service was while the insured was covered? I had a bill that was denied because the physician was thought to be out of network and was originally denied. After research, we discovered the physician was in network at the time of service, but is being denied because the policy has terminated since then.
