When you call give them your policy number for Southwestern General Life. If they are unable to locate it by policy number they will need the policy holder's name and date of birth. If they are unable to locate it with that information then you will have to photocopy the policy and any other documentation you have and send it to the address listed above. They will research the policy and get back with you. Good luck.
