provider billing incorrectly

I have a PPO plan. My son received services in a participating provider's office. My office benefits are Copay then 100%. This was billed as oph instead of clinic and my insurance company told me the provider's office needs to bill with place of service 11 and submit medical records for it to be paid at 100%, but the provider refuses to do this. Does anyone know who I can submit a complaint about the provider to?
