Florene, I forget in which order
the following Companies were acquired by Monumental Life Insurance Company, a division of AEGON, a Dutch multi-national conglomerate. Peoples Security Insurance, Public Savings Insurance, Commonwealth Insurance and DURHAM LIFE INSURANCE and Capital Security Insurance, a Providian Company were finally acquired by Monumental Life Insurance Company.
They are still in the Debit Insurance business and a local agent should be close by. Log onto http://www.monlife.com and there may be a web click button to locate an agent or you may be able to download their Claim Form.
