If you're in Ohio and are looking for individual coverage, two excellent choices are Anthem Blue Cross and Shield ( http://www.anthem.com ) -- however, with Anthem, if you have any medical history whatsoever, you will likely be declined --so, either lie on the application, or go with the second choice -- Supermed One -- http://www.supermedone.com
